Understanding Upper Blepharoplasty (Upper Eyelid Lift)

Upper blepharoplasty removes excess upper-eyelid skin and, when appropriate, a small amount of muscle or fat to restore a defined lid crease and a lighter, more open gaze. Incisions are placed within the natural eyelid fold and extend laterally as needed to address “hooding.” During consultation, brow position, true eyelid ptosis (levator function), skin quality, and tear-film health are assessed to design a plan that enhances eyelid definition while preserving natural contour and blink function.

Upper blepharoplasty is often paired with complementary treatments—such as brow or forehead lift for lateral hooding, internal ptosis repair for low lid height, and light laser or chemical resurfacing to refine fine lines—to achieve a refreshed, rested look.

Understanding Lower Blepharoplasty (Lower Eyelid Rejuvenation)

Lower blepharoplasty treats under-eye fullness and shadowing caused by age-related fat protrusion and ligamentous changes at the lid–cheek junction. Depending on anatomy, fat may be conservatively removed or repositioned over the orbital rim to smooth the tear trough; approaches include a transconjunctival incision (no external scar) for fat work or a subciliary incision with a limited “skin pinch” when extra skin or muscle laxity is present. Preoperative evaluation considers eyelid tone and canthal support, the distinction between true fat pads and fluid/festoons, and midface volume.

Lower blepharoplasty is frequently combined with adjuncts—such as canthopexy/canthoplasty for lid support, fractional laser or chemical peel for fine wrinkles, and selective midface/cheek augmentation—to create a smoother, more continuous transition from lower eyelid to cheek.
